1. Appointment + Form TLS France

2. Form Embassy France

3. Statement Letter (For children under 18 years old only)

4. Letter of Authority (Surat Kuasa) (Needed for visa application> file

5. Children Letter of Authority (Surat Kuasa Khusus Anak) (for Children under 18 leave without one parent / both parents)

6. Passport validity minimum 6 months from estimated arrival date.

7. Newest Photo with 3,5 x 4,5 size = 2 sheet, with white background, using photo paper with good quality and clear proportional face when zoomed 70%. Ears must be shown clearly on photo.

8. Sponsorship letters in English from company where applicant works. If there is more person/family joining the trip please put his/her name and status on the letter.

9. At sponsorship letter must include: Passport Number, Department rank, Salary per month

10. Copy of SIUP (Owner) and in English and NPWP must be added if the applicant is General Manager, Director, President Director or Commissioner

11. If the applicant is business owner and not having official letterhead then applicant can type the letter in English with company’s stamp and also attach copy of NPWP and SIUP(Owner) in English

12. If the applicant is getting sponsor from his/her child, please attach copy of child birth certificate. This can proves application relationship with the child. → In English

13. If the applicant is getting sponsor from in law, please attach copy of his child married certificate and birth certificate to prove the relationship → In English.

14. If the applicant is retired, the letter can be typed on paper In English and signed

15. Copy of applicant’s bank statements for last 3 months ( with clear name and account number, until last transactions) minimum IDR. 50 Millions/ person

16. Copy of salary slip for last 3 months

17. Copy of Family Card ( KK ) + Resident Identity Card ( KTP )

18. Copy of Marriage Certification ( Akte Nikah )

19. If applicant’s child joining the trip and still actively school, then you can add:

20. Copy of Student Card (Kartu Pelajar)

21. School letter of Confirmation (Surat Keterangan Sekolah Asli) in English

22. Copy of Birth Certificate (Akte Kelahiran) In English

23. Print out of Reservation Tickets

24. Hotel confirmation during trip on Europe

25. Traveling Insurance that valid for the duration of stay in Europe and money guarantee (uang pertanggungan) USD 50.000 or EURO 30.000. Travel Insurance is required to cover covid-19.

26. If there is any invitation from France and verified by local government, applicant must include copy of passport, CNI, and letter of invitiation from the invitor.

27. Form is filled and signed by applicant. For the child who doesn’t have Resident Identity Card (KTP), the form is signed by both of his/her parents.

• The vaccines received by france are:

• And the vaccine certificate must include:

• Name

• Type of vaccine

• Date of the final dose received

• Issuing country

• Issue organization
